Disabled Person Parking Permits
Individuals with temporary or permanent disabilities and transporting organizations may apply for a disabled person parking permit. A completed form HSMV 83039 Application for Disabled Person Parking Permit is required. The applicant and certifying authority or physician must have signed the application within the last 12 months. The application must include original signatures and the applicant’s valid Florida drive license/ID card or photocopy. A representative can mail or deliver applications. Representatives must have the applicant’s license/ID card or photocopy and the completed application with original signatures. Disabled persons must have either a current Florida Driver License or a Florida Identification Card.
Certifying medical authorities include:
- A physician licensed under Chapter 458 medical practice, Chapter 459 osteopathic medicine or Chapter 460 chiropractic medicine;
- A podiatric physician licensed under Chapter 461;
- An optometrist licensed under Chapter 463 (sight only);
- An advanced practice registered nurse licensed under Chapter 464, under the protocol of a licensed physician as stated above;
- A physician assistant licensed under Chapter 458 or Chapter 459; or
- A similarly licensed physician from another state if the application is accompanied by documentation of the physician’s licensure in the other state and form HSMV 83039 signed by the out-of-state physician verifying his or her knowledge of this state’s eligibility guidelines.
Disabled person parking permits:
- Must only be used by the permit holder. The permit holder must have their Florida driver license or ID card on-hand when using the permit.
- Must be hung from the rear-view mirror, with the permit number visible from the front of the vehicle, when the vehicle is parked. The permit should not hang from the rear-view mirror when the vehicle is in motion.
- Must have a valid, yellow registration decal attached to both sides of the permit.
- Must display on one side, the permit holder’s driver license or ID card number, or the authorized organization’s FEID number.
Permanent Parking Permit (blue card)
Persons with long-term mobility impairments, or who are certified as legally blind, qualify for a permanent disabled person parking permit. Permanent permits are valid for four years and must be renewed by the permit holder’s birthdate. There is no fee for this parking permit. Renewals require a new HSMV 83039 Application for Disabled Person Parking Permit signed by the applicant and certifying physician or authority. Individuals are limited to two placards.
Temporary Parking Permit (red card)
Persons with short-term mobility impairments may receive a temporary disabled parking permit for up to six months as specified by a certifying medical authority. Temporary disabled person parking permits require a $15 fee. If the need for the temporary disabled person parking permit exceeds six months, an additional temporary permit may be issued. If the additional permit is applied for within 12 months from the issuance of the first permit, no additional fee is required.
Replacements
A replacement (lost or stolen) permanent permit requires a form HSMV 81346 Application for Replacement and a new HSMV 83039 Application for Disabled Person Parking Permit if it has been more than 12 months since the original document was signed.
Disabled Person Wheelchair License Plate
Persons with long-term mobility impairments, or who are certified as legally blind, may request a wheelchair license plate be issued for a motor vehicle registered in their name, in lieu of receiving a permanent disabled persons parking permit. The fee for a wheelchair license plate is $28 in addition to other applicable registration fees. Wheelchair license plates must be renewed annually on the applicant’s birthdate. The long-term impairment must be proved every four years by a certifying medical authority.
Wheelchair license plates may only be issued for use on the following types of vehicles:
- Automobiles for private use or lease.
- Trucks weighing 5,000 pounds or less.
- Automobiles, which seat fewer than nine passengers and are for hire.
- Motor homes or truck campers not for hire or commercial use.
- Heavy trucks with a gross vehicle weight of less than 8,000 pounds.
Certifying medical authorities include:
- A physician licensed under Chapter 458, Chapter 459 or Chapter 460;
- A podiatric physician licensed under Chapter 461;
- An optometrist licensed under Chapter 463 (sight only);
- An advanced practice registered nurse licensed under Chapter 464, under the protocol of a licensed physician as stated above;

Disabled Veteran License Plate
Veterans who are confirmed 100% disabled from a service-connected disability by a certifying medical authority are eligible for a disabled veteran license plate at no charge. Disabled veterans must complete form HSMV 83039 Application for Disabled Person Parking Permit. The veteran must provide proof of service-connected 100% disability rating or that the vehicle was acquired through financial assistance from the Department of Veterans Affairs.
